Smokers And Lipstick Lines

Smoker’s lines, also known as lipstick lines or perioral rhytides, are the fine vertical creases that appear above and below the lips. They are caused by repetitive lip movement, reduced collagen, and thinning of the skin. The name is misleading, many people who have never smoked still develop them.

They are caused by repetitive lip movement, reduced collagen, and thinning of the skin. The name is misleading, many people who have never smoked still develop them.

  • Smoker’s lines affect both men and women, typically from the mid-thirties onwards. They are common in people who smoke, use straws frequently, or have strong orbicularis oris muscle activity. People with fair, thin skin or significant sun exposure are more prone to early onset.

    • Fine vertical lines above and below the lips

    • Flattened vermilion border

    • Lipstick “bleeding” beyond the natural lip line

    • Downturned corners of the mouth (marionette effect) in advanced cases

  • There is no NICE guidance for perioral vertical lines (“smoker’s lines”). The BNF gives general toxin guidance but does not address this indication.

  • A full consultation determines suitability, assessing medical history, allergies, previous filler use, and lifestyle factors such as smoking or sun exposure. 


    Injectable treatments are contraindicated during pregnancy, breastfeeding, or if there is an active cold sore. 


    All prescription treatments follow Regulation 12 (Safe Care and Treatment) and are carried out under direct prescriber oversight with consent documentation and cooling-off periods.

    • Botulinum toxin: results visible within 5–10 days, lasting around 3–4 months.

    • Fillers: immediate improvement, settling over 1–2 weeks, lasting 9–12 months.

    • Peels or microneedling: gradual improvements over several weeks as new collagen forms.Patients often notice smoother lip texture, improved lipstick application, and a more defined lip contour without exaggerated volume.

    • Avoid touching, massaging, or applying lipstick for 24 hours post-treatment.

    • Keep the area clean and hydrated.

    • No smoking, alcohol, or strenuous exercise for 24–48 hours.

    • Use SPF daily to protect collagen integrity.

    • Maintain results with top-ups every 6–12 months, depending on treatment type.
