Why do we provide them?
To explain clearly what each treatment involves.
To outline the benefits, risks, and possible side effects.
To give you details of any alternatives available, including doing nothing.
To ensure you understand the aftercare advice needed to keep you safe.
This helps you make a decision that is right for you, without pressure or obligation.

You can take as much time as you need to read the information sheets before agreeing to treatment.
You can ask us for more information if anything is unclear.
You can change your mind at any point, even after signing the form, and we will respect your decision.
