Why do we provide them?

• You can take as much time as you need to read the information sheets before agreeing to treatment.

• You can ask us for more information if anything is unclear.

• You can change your mind at any point, even after signing the form, and we will respect your decision.

If you feel that the consent sheet you are given does not explain enough, or if you would like extra details, please:

• Ask your treating clinician to explain further in person.

• Request additional supporting information (we can provide evidence-based guidance, such as NICE or MHRA advice).
